You must return the following three items. The deadline for returning them is the day specified on the Enrollment Confirmation Instructions included in the large white envelope.

  • Print, complete, and sign the New Student Payment Form.
  • From the Enrollment Confirmation Instructions, remove the lower portion, called the Enrollment Confirmation Form, and review address information at lower left. Keep the Instructions for your records—it indicates your UW Student Number and other important information—and return the Form.

Due Dates

  • The three items listed above must be returned by the date specified on the Enrollment Confirmation Instructions.
